How Do You Start a Behavioral Health Agency in Georgia?

To serve DBHDD and Medicaid clients in Georgia, you usually go through a DBHDD enrollment process, then apply with DCH for Medicaid. Many services also need a state license, such as a DATEP license, and national accreditation. Rules depend on the services you plan to offer, so confirm each step with DBHDD and DCH.

Start with your service model

Before any form, decide what you will offer and who you will serve. Adults or youth? Mental health, substance use or both? Office-based or in the community? Your answers decide which licenses, staff and rules apply.

The DBHDD Provider Manual for Community Behavioral Health Providers defines each service and its staffing and documentation rules. Read the sections for the services you want. The manual is updated every quarter.

The DBHDD provider process

DBHDD's "Become a Provider" page lays out two phases.

Phase 1:

  1. Register for and attend a New Provider Enrollment Forum run by the Georgia Collaborative ASO.
  2. Submit a Letter of Intent (LOI) during the open enrollment period.
  3. If the LOI is approved, you get an invitation to apply.

Phase 2:

  1. Submit the DBHDD New Provider Application.
  2. If required, a Regional Field Office does a site visit.
  3. After DBHDD approves, you complete the online DCH application.

DBHDD says that if DCH denies the application, you must wait one year and start over at Phase 1. Enrollment periods open and close, so watch the Georgia Collaborative site for dates.

Licensing: the DATEP question

Georgia rule 82-10 says a Drug Abuse Treatment and Education Program cannot operate without a license or provisional license. DBHDD's current application packet says applications are filed online and that the first review is done within 30 business days.

The packet lists items such as Secretary of State registration, proof you own or lease the property, a fire safety inspection dated within 12 months, a certificate of occupancy, zoning approval, a floor plan and background checks for all owners.

DBHDD is the agency that licenses DATEPs. A Georgia law, HB 584, moved this job from DCH to DBHDD on January 1, 2026. DBHDD says you may still be sent to DCH websites for some forms or payments for now, and those get passed on to DBHDD. The rule also says that, starting July 1, 2026, new branch programs need their own license.

Accreditation

The DBHDD Provider Manual tells providers to follow DBHDD policy 01-103, Accreditation and Standards Compliance Requirements. Some service definitions in the manual say the agency must be CARF or Joint Commission accredited. Rule 82-10 also lets the licensing department accept accreditation reports when it issues a license.

Accreditation can take a long time. CARF says getting ready can take a year or more. Plan for it early.

Medicaid enrollment

Georgia Medicaid providers enroll online through the GAMMIS Enrollment Wizard. Approved providers get a Medicaid provider number. DCH also runs one credentialing process through a Credentialing Verification Organization that shares decisions with the state's managed care plans. See our Medicaid article for more.

Plan your money and time

Every step takes time, and payments do not start the day you open. Build a budget that covers rent, staff, insurance, an EHR and the months before steady payments. DBHDD's policy 21-101 covers financial and reporting rules for community providers. Read it early.

Frequently Asked Questions

Do I need a license to open an outpatient therapy agency in Georgia?

It depends on your services. A Drug Abuse Treatment and Education Program needs a license from DBHDD under rule 82-10. Individual clinicians also need their own professional licenses. Ask DBHDD which licenses apply to your plan.

How long does it take to become a DBHDD provider?

There is no single timeline. It depends on enrollment periods, your application, any site visit and DCH review. Plan for many months, not weeks.

Do I need accreditation before I apply?

Some DBHDD services require CARF or Joint Commission accreditation. Check the service definitions in the current Provider Manual and policy 01-103, and ask DBHDD about your specific services.

What happens if DCH denies my application?

DBHDD says you must wait one year after the denial and start again at Phase 1 in a later enrollment period.
